Cheshire East’s Neighbourhood Care Model Decreases Emergency Room Visits

A neighbourhood care initiative implemented across eight communities in Cheshire East has successfully reduced A&E attendances by as much as 48%. This data-driven program focuses on early intervention to support residents at risk of health crises.

Key Achievements of the Programme:
  • 14.6% reduction in A&E attendances.
  • 26% decrease in emergency admissions among high-risk residents.
  • Financial savings of up to £2.8 million in secondary care costs.

Between November 2024 and November 2025, the program proactively supported 3,587 high-risk individuals through multidisciplinary neighbourhood teams. Dr. Anushta Sivananthan, a consultant psychiatrist and senior officer for integrated neighbourhood teams, emphasized the importance of empowering local teams to work proactively with at-risk residents.

Impact on Patient Wellbeing:

The initiative has not only reduced hospital visits but also improved the overall wellbeing of participants. Notable outcomes include:

  1. An 83-year-old man received rapid physiotherapy and befriending support, enhancing his mobility and mood.
  2. An 81-year-old woman with diabetes and heart failure benefited from coordinated referrals, improving her independence.
  3. A 61-year-old woman with obesity saw improvements in her wellbeing after receiving household support and social prescribing.

Despite a 5% increase in the Cheshire East population, the demand for urgent and emergency care has not risen proportionately, indicating the effectiveness of this proactive neighbourhood management approach. The program utilizes the CIPHA platform for population segmentation and predictive modeling, identifying over 2,150 residents in need of enhanced care.

Cheshire East is now recognized as a leader in the region for its innovative approach to community health and care delivery.
