Healthcare leaders are rushing to deploy agentic AI. These are not simple chatbots. They are autonomous agents designed to make clinical and operational decisions. But speed has overtaken safety.

A staggering 72% of healthcare organizations are deploying these autonomous tools without formal IT approval. This is “shadow AI” on a massive scale. While 28% of health systems already have agentic AI in production, and another 44% are running pilots, governance is an afterthought.

Most executives expect these tools to operate with high autonomy. They want to streamline workflows and cut administrative bloat. But deploying autonomous software without IT oversight is a recipe for disaster.

The Identity Crisis

If an AI agent can prescribe a workflow or access patient records, who is accountable? Currently, nobody knows. Over half of industry leaders rank security as their top anxiety. Unapproved tools risk exposing sensitive patient data and leaking proprietary code.

This is not just about data privacy. It is about clinical integrity. If an unauthorized agent alters a patient record, the legal and medical liability is unprecedented.

The solution is not banning the technology. It is treating AI agents like employees.

Hospitals must implement identity-led governance. This means giving every autonomous agent its own digital credentials. If an AI action cannot be tracked to a specific, authorized identity, it should not run.

Until health systems bridge this governance gap, they are practicing medicine in the dark. The risk is no longer just a system crash. It is a direct threat to patient safety.
