Welsh Hospitals End Paper Prescribing to Cut Errors

Replacing paper medication charts with unified digital prescribing is reducing transcription errors and administrative friction across rural Welsh hospitals.

Replacing paper medication charts with unified digital prescribing is reducing transcription errors and administrative friction across rural Welsh hospitals.

Paper-based medication records create dangerous friction points whenever a patient transfers between care settings. Powys Teaching Health Board has phased out paper charts by deploying an electronic prescribing and medicines administration system at Bronllys Hospital, marking a key step in Wales’s national Digital Medicines programme.

Connecting Patient Histories

The rollout connects local clinical workflows directly to a national Shared Medicines Record. Clinicians now read admission histories and issue discharge prescriptions through a single consolidated database. This eliminates manual re-entry of drug regimens between community hospitals and inpatient services.

Removing paper charts addresses a long-standing vulnerability in patient transfers. Transcribing errors drop significantly when medication histories follow the patient automatically across health board boundaries.

Infrastructure Dependencies

However, digital prescribing tools only prevent harm if clinical teams adopt standardized workflows across every department. Initial success at a community hospital setting does not automatically guarantee seamless cross-regional performance without continuous training and reliable infrastructure.

Health systems evaluating prescribing software must integrate national patient data repositories rather than creating isolated digital silos.
