Introduction
Graphnet Health has been chosen by Jersey’s Family Nursing and Home Care (FNHC) to implement the island’s first extensive remote monitoring service.
Growing Elderly Population
- The population of islanders aged 65 and older is projected to rise by 30% by 2040.
- There will be a 50% increase in the number of individuals living into their 80s and beyond, as per statistics released in December 2023.
Remote Monitoring Solution
Under this contract, FNHC will utilize Graphnet’s remote monitoring solution, powered by Luscii, to monitor the health of frail and vulnerable residents both at home and in community settings.

Funding and Implementation
The initiative is funded through the Impact Jersey CareTech Challenge program, with a grant of nearly £800,000 awarded to FNHC for the rollout of a digital monitoring system for older residents in Jersey.
How the System Works
Using the Graphnet remote monitoring app and connected devices, patients and caregivers can:
- Record essential health metrics at home, including blood pressure, weight, and wound images.
- Allow FNHC clinicians to access this information in near real-time and receive alerts if any readings fall outside of established parameters.
Project Timeline
Starting in January 2026:
- The first year will concentrate on frailty and falls, supporting 100 individuals with remote monitoring at any given time.
- The second year will expand to include pathways for diabetes and wound management, assisting an additional 100 individuals.
