Automated insulin delivery is moving rapidly into Type 2 diabetes care, shifting daily management from manual dosing to algorithmic control.
Millions of adults with Type 2 diabetes who rely on daily insulin injections now have regulatory clearance to use automated pumps previously reserved largely for Type 1 patients. The FDA expanded clearance for the Twiist automated insulin delivery system to cover adults with Type 2 diabetes, expanding potential access to 5.5 million people in the United States.
Clinical Impact
Algorithms adjust delivery automatically based on continuous glucose monitoring. In a 307-patient pivotal trial supporting the decision, participants gained an extra 3.8 hours per day in their target blood sugar range. Average HbA1c fell by 0.7% overall, with high-risk patients starting above 9% seeing a 2.1% drop.
Hardware flexibility matters for real-world adoption. The device connects directly with glucose monitors like the FreeStyle Libre 3 Plus and Eversense 365-day sensor, with direct control available through an Apple Watch. Pharmacy distribution further lowers barriers compared to traditional durable medical equipment channels.
Practical Limits
Clinical trial results do not guarantee immediate real-world outcomes. Algorithmic dosing reduces patient burden, but daily interaction and sensor management remain mandatory. Provider training and insurance coverage will dictate how fast clinical practice shifts away from manual injections.
