Specialty medications like GLP-1s and complex biologics now command over 60 percent of prescription spending for many health plans. Managing these high-cost therapies is a logistical nightmare of prior authorizations, cold-chain shipping, and clinical monitoring. Every delay threatens patient adherence and drives up administrative costs.

The $100 Million Bet

Evernorth is committing $100 million through 2028 to deploy its AI-powered Pharmacy Forward program. Operating through its Accredo specialty pharmacy, the initiative targets the administrative friction that delays patient care.

The goals are highly specific. Evernorth aims to cut prescription processing times in half and slash clinician administrative workloads by up to 50 percent. For patients, the target is same- or next-day delivery for 90 percent of orders.

This is a massive operational gamble. Specialty pharmacy is the primary profit engine for modern pharmacy benefit managers. When administrative delays happen, patients abandon therapy, and insurers lose money.

The Realist Outlook

This is a defensive maneuver. Competitors like CVS, Humana, and Elevance Health are pouring capital into the lucrative specialty pharmacy sector. They all want to capture the high-margin coordination of complex care.

But software cannot solve the underlying crisis of drug pricing. While predictive logistics might get a vial to a patient’s door faster, it does not lower the astronomical wholesale cost of the therapy itself. AI can streamline the plumbing, but it cannot shrink the bill.
